• Post Grant Report Form

  • Post Grant Report Forms must be completed for the year the funds were received, prior to eligibility for a subsequent grant. Organizations who do not submit a Post Grant Report Form after receiving a grant, will be ineligible for further funding until the conditions of funding are completed.

    2. Financial report.
    3. Proof of payment, invoices, receipts, cancelled cheques, etc.
    4. Photos and stories about the grant supported event.
